Following are the ways in which ExperTox can provide services for your individual or company needs:

4. To request testing services or supplies for collections, forms, etc. either contact ExperTox and give type and quantity of specimen collections containers required or download the form from our web site and fax to ExperTox at 281-930-8856. Please download the form here.
5. Collection protocols for the following tests and procedures:

a. Download the Custody and Control Requisition Form.
b. Fill out the information on the Requisition for testing requirements and type of specimen or evidence.
c. Please be sure to provide name, address, phone, fax and email information for communication and result of analysis reporting.
d. Include payment in the form of Money Order or Cashier's Check.
